Student Forms Checklist
In order to comply with NH Law and to provide you with quality health and wellness services while your child attends St. Paul's School, all health-related forms must be completed prior to your child's arrival at St. Paul's School. Please use the checklist provided to help you organize what forms are required. All forms are due by June 15.

Applicable At-Risk Students

Vaccine Information Statements (VISs) are information sheets produced by the Centers for Disease Control and Prevention (CDC) that explain to vaccine recipients, their parents, or their legal representatives both the benefits and risks of a vaccine.

- * Diphtheria Tetanus and Pertussis
HPV (Human Papillomavirus)
Provide written reports from medical/psychological providers:
May include: psychological testing or reports to support a diagnosis of ADHD; reports relating to extended time testing or educational accommodations; or, medical reports following a serious injury/illness with recommendations for follow-up care while attending St. Paul's School.
